Reporting of changes
Please provide a timely report on all changes to the client department of ASEKOL, which has the duty to inform the MoE of all changes in the entry in the List of Producers of EEE.
Overview of changes and documents:
Change or extension of categories and sub-categories – please provide Annex No. 1 to the Joint Performance Agreement (pdf, 27 kB) executed by the statutory body of the company and the completed and executed form Identification of the producer (doc, 94 kB) including specification of all applicable categories and sub-categories. In case of a producer in Category No. 8, it is also necessary to furnish an affirmation confirming that the relevant products are not intended for private households. To get the form contact our customer department.
Change in the registered office, legal form or name of the company – please send us an original counterpart or an authenticated copy of an extract from the Commercial Register not more than 3 months old.
Change in the statutory body (if the Id. No. remains unchanged) – please send us a copy (authentication not required) of an extract from the Commercial Register not more than 3 months old and a completed Identification of the producer (doc, 94 kB) form with specification of all the applicable categories and sub-categories executed by the new statutory body.
Change in the mailing address – please provide the relevant information either in writing or by sending an e-mail to our client department.
Change in the manner of labeling EEE – please complete the form Manners of labeling electrical and electronic equipment (doc, 752 kB) form.
Supplementary registration of a new product – please furnish the completed and executed form Identification of the producer of electrical and electronic equipment (doc, 94 kB) and the form Manners of labeling electrical and electronic equipment (doc, 752 kB). Change in Id. No. - please furnish newly executed agreements, including all annexes, newly furnish all registration documents to the Ministry of the Environment (i.e. an extract from the Commercial Register, either the original counterpart or an authenticated copy, not more than 3 months old, a power of attorney, consent, identification of the producer of EEE and the manners of labeling EEE). Termination of activities – please furnish a notice to our collective scheme and an affirmation. Please always contact our client department!
