New registration
To participate in the system, it is necessary to:
1. conclude an agreement with the ASEKOL s.r.o. collective scheme
2. lodge, through the ASEKOL collective scheme, an application for registration in the List of Producers of EEE kept by the MoE

Annexes No. 1, No. 2 and No. 3 are part of the Agreement. To receive contributions from the current tariff (Appendix 2) contact the client department. All documents have to be signed in Czech version, available here. Please examine all contractual documents and complete, execute and send two original counterparts to ASEKOL s.r.o., Československého exilu 2062/8, 143 00 Praha 4.
Do not send us Annexes No. 2 – Tariff of Fees – and No. 4 – Logo of the Producer!
2. The following documents must be supplied for registration of the producer/importer resulting in registration in the List of Producers of EEE kept by the MoE:
- Extract from the Commercial Register – the original counterpart or an authenticated copy not more than 3 months old (if the producer is registered in the Commercial Register)
- Trade and concession certificates – authenticated copies (only if the producer is not registered in the Commercial Register)
- Power of attorney (doc, 38 kB – with the officially authenticated signature of the statutory body of the producer (for persons registered in the Commercial Register, the manner of acting /signing must correspond to the entry in the Commercial Register)
- Consent (doc, 38 kB) – with the officially authenticated signature of the statutory body of the producer
- Identification of the producer (doc, 94 kB) – executed by the statutory body of the producer (the signature need not be officially authenticated)
- Description of the manner of labeling products put on the market (doc, 752 kB)
- Affirmation (doc, 30 kB) - if not a branch office in Czech Republic (the signature doesn't need to be authenticated officially).

Please send the completed documents, together with the agreements, to ASEKOL s.r.o. Československého exilu 2062/8, 143 00 Praha 4.
